County Line Chiropractic Center, Inc. (“County Line,” “we,” “us,” or “our”) operates countylinechiro.com. This policy explains what information this website collects, how we use it, and what choices you have.

What this policy does and does not cover

This policy covers this website only.

It does not cover the medical records we keep about patients we treat. Those records are protected health information under HIPAA and are governed by our Notice of Privacy Practices, which you receive at your first visit and can request from any of our clinics at any time.

Nothing on this website is a patient portal, and we do not ask you to submit medical information through this site.

If you are already a patient and have a question about your records, contact the clinic directly rather than using this website.

Information you give us

Our contact form asks for your name, email address, phone number, and which of our clinics you are interested in. That is all it asks for.

Please do not send us medical details through this website. We will ask about your accident and your symptoms when we speak with you by phone or when you come in — those conversations are protected in ways that a web form is not.

Form submissions are delivered by email to County Line staff. We do not store them on this website.

When you submit the form, hashed (scrambled) versions of your name, email address, and phone number are also sent to Google, which uses them to determine whether our advertising led you to contact us. Google is contractually restricted in how it may use this information, and it is not used to build advertising audiences based on health conditions. We do not send your form submission to any other advertising or analytics company.

Along with your form submission, we receive information about how you found us — such as the search or advertisement you clicked. We use this to understand which of our marketing efforts are working.

Information collected automatically

Like most websites, ours uses cookies and similar technologies that collect information automatically when you visit. This includes your IP address, device and browser type, the pages you view, the site or ad that referred you, and how you move through the site.

These tools currently load when you open a page on our site. We are in the process of adding controls that will let you decide which of them run before they load. This policy will be updated when that changes.

• We do not ask for or want medical information through this website.

• To respond when you contact us and to schedule an appointment

• To understand how people find and use our website, so we can improve it

• To measure whether our advertising is reaching people who need our services

• To keep the site secure and working properly

Who we share information with

• Service providers who operate the site and our systems on our behalf, including our website platform (Framer) and our email provider (Microsoft), each bound by contract to protect what they handle

Children

This website is not directed to children under 13, and we do not knowingly collect personal information from them. If you believe a child has submitted information through this site, contact us and we will delete it.

Where we operate

County Line Chiropractic Center, Inc. is a Florida profit corporation operating exclusively in the State of Florida. This website is intended for residents of Florida and the surrounding area. If you access it from elsewhere, you do so on your own initiative.
